Injury of bowel during insertion of first trocar in patients after previous laparotomy

The aim of this study was retrospective evaluation of bowel injury during transumbilical first trocar insertion in patients after previous laparotomy. 1008 laparoscopic interventions performed in the Academic Hospital of Free Berlin University in Potsdam between 1994 and 2001 were analysed. All evaluated patients had previously had one or more abdominal operations. In 2 patients (0.2%) the intestinal wall had been injured. In patient 1 the large intestine and in 2 patient the small intestine had been damaged. In both cases the damage had been treated with laparotomy. After evaluation of both cases, in 2001 open laparoscopy was introduced in all patients with previous abdominal interventions. There has been no incident noted of bowel injury since then.
