Introduction:
Mastocytosis is a heterogenous group of myeloproliferative neoplasm, which is characterized by excessive proliferation and accumulation of mast cells in organs. Cutaneous mastocytosis is a type of mastocytosis that is most common in children and has different clinical presentations.

Objective:
Our aim was to present the atypical presentation of cutaneous mastocytosis in infant.

Case report:
A 9-month-old girl was admitted with diffuse erythematous lesions with blisters and erosions, which appeared 4 months before the admission. The patient was treated unsuccessfully with antibiotics due to suspected bacterial infection. Clinical examination revealed a positive Darier's sign and laboratory work detected. A skin biopsy revealed mast cell dermal infiltrate confirmed by immunohistochemistry staining corresponding to cutaneous mastocytosis.

Conclusions:
Cutaneous mastocytosis can have different clinical manifestations, which can give a potential diagnostic difficulty and prolong the time of diagnosis.
