Disseminated superficial actinic porokeratosis. Two case reports

Porokeratosis is a rarely occurring genodermatosis in which disturbances in keratinization are believed to play the main role in the pathogenesis. Among some clinical forms of porokeratosis, disseminated superficial actinic porokeratosis seems to be one of the most frequently occurring. A typical histological phenomenon for all types of porokeratosis is the presence of cornoid lamella which creates a parakeratotic column. We present a case of a 33 y.o. man and a 65 y.o. woman of Caucasian origin who presented with disseminated annular lesions with slight keratinization, located on the trunk, buttocks and limbs. In the lesions’ centres atrophy was observed. Based on the medical history, enhanced exposure to ultraviolet radiation (UVR), being one of the aetiological factors in porokeratosis, was revealed in the two presented patients. The patients did not use any rational forms of photoprotection. In the two patients skin biopsies were taken. Based on medical history, clinical picture and results of histological examination, disseminated superficial actinic porokeratosis was diagnosed. We proved the role of enhanced exposure to UVR in development of actinic porokeratosis, which indicates the necessity of continuous use of photoprotection. In the article diagnostic and therapeutic difficulties in actinic porokeratosis are highlighted.
