Diagnostic and therapeutic problems in early glottic cancers – a case report and overview of the literature

The case of a 73-year-old male patient with left vocal cord tumour is presented. The tumour grade was evaluated as T2bN0M0; the mobility of the vocal cord was conserved; the infiltration embraced the anterior commissure and subglottic region. Squamous cell carcinoma was confirmed in histology. Primary radiotherapy (RT) was advocated by the oncological team. In the process of RT planning the thyroid cartilage infiltration in computed tomography (CT) was revealed; thus the patient was disqualified from RT. After reconsultation of the oncological team the patient was directed to reconstructive laryngectomy with cricohyoidoepiglottopexy. During the operation the pyramidal lobe of the thyroid gland in the region of thyroid cartilage lamina was found and the lamina of thyroid cartilage was found to be free from the neoplastic infiltration. We present this case because of the discrepancy in laryngological and CT examination results. We discuss patient-qualification stages by the oncological team according to algorithms.
