Information on fees
NOTE: the fee amount depends on the date of payment, NOT the date of registration.By 30 June 2015: PLN 290 After 1 July 2015: PLN 400 Paid upon arrival: PLN 600 The fees include VAT.  Termedicus Club members get up to 40% discount on the participation fee. The conference package includes the following: – participation in conference sessions, – detailed programme of events, – conference materials, – certificate of attendance, – coffee. To take part in the Conference, you need to: – register your attendance, – pay the conference fee to the publishing house’s bank account, – accept the terms and conditions of participation. In accordance with procedures followed by the Publishing House, we limit the issue of corrective invoices, so please make sure that the invoice data are complete and final. To register for the Conference: – go to the website www.termedia.pl, – send an e-mail with the Registration Form to: szkolenia@termedia.pl Registration Form – send a fax to +48 61 656 22 00 (please quote the name of the conference, full name of the participant, correspondence address, e-mail address, medical specialty, telephone, NPWZ and complete invoice data). Deadline for accepting payments by bank transfer: 4 December 2015. Cancellation of attendance must be made in writing (by post, fax, e-mail) by 27 November 2015. After that date, paid fees will not be refunded. Participants making payments upon arrival cannot be guaranteed a full conference package. |
