Surgical treatment of eyelids tumors

Evaluation of surgical treatment of tumors of the eyelids combined with diathermocoagulation of blood vessels and cryotherapy. 99 patients with malignant tumors located at the eyelids or in the area of medial canthus or lateral canthus of the eye were operated. There were 51 men and 48 women aged 27 to 83 years [average age was 56,1 year]. Each tumor was excised, when necessary with a margin of normal skin and tarsus and conjunctiva. Diathermocoagulation was applied before each incision. The bleeding vessels were closed by diathermocoagulation, superficial and deep margins were frozen at -60oC for 30 seconds. Surgical repair was dictated by defect: in 11 cases with large defect tarsoconjunctival flap of the upper lid of the same or the opposite eye was fashioned together with skin rotated flap or with the graft of the skin taken from the arm. In 9 cases of large defects a skin graft from the arm was transplanted, in 7 cases rotation of lid margin with lateral angle incision was performed. In 72 cases a skin flap was rotated.

Histopathological examination revealed basal cell carcinoma in 90 cases, squamous cell carcinoma in 5 cases, melanoma in 3 cases and adenocarcinoma in 1 case. In the majority of cases [78%] good functional and aesthetic results were obtained; in 17% cases eyelid function was restored, in 4% compromised eyelid functions resulted. In 4 cases a local recurrence developed.
