Surgery remains the cornerstone of treatment for women with a diagnosis of severe urodynamic stress incontinence and also in those who have failed to improve with conservative measures. In this article un update of current diagnostic algorithm as well as clinical implication of proper diagnosis of female stress urinary incontinence has been presented. Moreover current knowledge concerning clinical safety and effectiveness of midurethral sling procedures (retropubic as well as transobturator) was presented. Additionally two new surgical ”kits” (TFS and TVT Secure) recently introduced into clinical practice were described.
