Introduction:
Squamous cell carcinoma is the second most common type of skin cancer in humans, with a notable rise in cases in recent years. The occurrence of bullous pemphigoid in association with squamous cell carcinoma is an exceptionally rare phenomenon.

Case report:
The paper presents the case of a 62-year-old male patient who exhibited non-pruritic erythematous and edematous lesions on the trunk. Direct immunofluorescence from the perilesinal skin showed linear deposits of IgG4 and C3 at the dermal-epidermal junction. Physical examination revealed extensive ulceration on the scalp, infiltrating the frontal bone. Based on histopathology and additional imaging tests, the diagnosis of cT4N0M0 squamous cell carcinoma was made.

Conclusions:
Various malignancies can present with bullous pemphigoid, but the paraneoplastic significance of this association remains unclear. The co-occurrence of bullous pemphigoid and squamous cell carcinoma strongly supports a paraneoplastic association.
