Introduction:
Paraneoplastic syndromes are clinical manifestations associated with cancer which are not a direct consequence of malignancies. Chronic spontaneous urticaria is characterized by recurrent whealing of the skin for at least 6 weeks with ambiguous etiopathogenesis.

Objective:
We present a very rare case of the paraneoplastic syndrome in the form of urticaria in gastric cancer.

Case report:
A 61-year-old patient presented to the physician because of urticaria occurring at nights in various locations. An irregular ulceration was detected in the gastric body; being diagnosed as gastric cancer. The patient received 4 cycles of preoperative chemotherapy with the FLOT regimen, followed by a subtotal distal gastrectomy (Billroth II), and 4 cycles of postoperative chemotherapy. After that, the urticaria resolved.

Conclusions:
The described clinical situation shows a clear association between the occurrence of chronic urticaria and gastric cancer. No description of a similar clinical case was found in the available literature.
