Trismus is a rare symptom of the nasopharyngeal cancer diagnosed in all of 5% of patients. It occurrence provides the most frequently infiltration by the tumor or masseter muscles. It arises as a tonic spasm of the masseter muscles due to infiltration motor fibers of the trigeminal nerve. Symptoms are usually diagnosed instantly by the patient and arousing his anxiety. It presented a case of 20 year-old woman with advanced nasopharyngeal and oropharyngeal adenocarcinoma. Despite of severe mouth opening lasting more than a year patient didn’t seek medical help to explain the cause of this disease.
