The article contains a theoretical and clinical analysis of the case of a patient with Korsakoff syndrome, which occurred in the second month of the complicated recovery process after bariatric surgery in a sleeve gastrectomy procedure. Korsakoff syndrome caused by thiamine deficiency was preceded by a Wernicke’s encephalopathy phase. After resolution of consciousness disorders, a neuropsychological assessment was performed and revealed severe anterograde and retrograde amnesia as the main clinical manifestations. Furthermore, discrete executive deficits and limited self-awareness of disease symptoms were observed. General intelligence was maintained at an average level. Thiamine supplementation and cognitive rehabilitation carried out for several months did not give expected therapeutic results allowing the patient to return to independence in everyday functioning. The described case confirms irreversibility of amnesia if Korsakoff syndrome occurs.
